Health Declaration Form (Visitors / Contractors / Service Providers)

GENERAL RULES FOR VISITORS / CONTRACTORS / SERVICE PROVIDERS

1. Visitors / Contractors / Service Providers have to submit Health Declaration Form 1 to 3 days before the entry date (subject to the approval by Head of Administration & Human Resource Division).
   
   Pelawat / Kontraktor / Penyedia Perkhidmatan perlu menghantar Borang Pengisytiharan Kesihatan 1 hingga 3 hari sebelum tarikh masuk (tertakluk kepada kelulusan oleh Ketua Divisyen Pentadbiran & Sumber Manusia).



2. The validity of Health Declaration Form is 14 days from the submission date however Visitors / Contractors / Service Providers will have to fill up the Health Declaration Form again should they have any changes to the information.
   
   Tempoh sah Borang Pengisytiharan Kesihatan adalah 14 hari dari tarikh penghantaran namun Pelawat / Kontraktor / Penyedia Perkhidmatan perlu mengisi Borang Pengisytiharan Kesihatan semula sekiranya terdapat perubahan pada maklumat pengisytiharan.



3. Body temperature screening will be conducted by security. Visitors / Contractors / Service Providers are required to record the temperature in the log book before entering the premise.
   
   Pemeriksaan suhu badan akan dilakukan oleh pengawal keselamatan. Pelawat / Kontraktor / Penyedia Perkhidmatan diminta untuk mencatat suhu di buku log sebelum memasuki premis.



4. Visitors/ Travellers travelling from countries which are presently affected with serious communicable or highly transmissible diseases such as COVID-19, Influenza or Monkey Pox are advised to do health screening prior to embarking on your journey to Malaysia (Please upload your Health Screening Result). Visitors / Contractors / Service Providers with high fever (37.5 ͦC and above) or exhibit any symptom of COVID-19, Influenza, Monkey Pox or any other transmissible diseases will be prohibited from entering our premise.
   
   Pelawat / Pengembara yang mengembara dari negara yang pada masa ini terjejas dengan penyakit berjangkit yang serius atau sangat menular seperti COVID-19, Influenza atau Monkey Pox dinasihatkan untuk melakukan pemeriksaan kesihatan sebelum memulakan perjalanan anda ke Malaysia (Sila muatnaik keputusan pemeriksaan kesihatan anda). Pelawat / Kontraktor / Penyedia Perkhidmatan yang mengalami demam panas (37.5 ͦC dan ke atas) atau menunjukkan sebarang simptom COVID-19, Influenza, Monkey Pox atau sebarang penyakit berjangkit lain tidak akan dibenarkan memasuki premis kami.



5. Individual under Home Surveillance Order is not allowed to enter the premise.
   
   Individu di bawah Home Surveilance Order tidak dibenarkan memasuki premis.



6. Visitors / Contractors / Service Providers are required to wear face mask and sanitise hand before entering our premise and maintain physical distancing.
   
   Pelawat / Kontraktor / Penyedia Perkhidmatan dikehendaki memakai pelitup muka dan membersihkan tangan sebelum memasuki premis dan mematuhi penjarakkan fizikal
